2.407 Liability Waivers
I. Overview
When an inherent or potential hazard and/or dangerous condition exists in an extramurally sponsored activity, appropriate rules of conduct should be adopted, posted, and, if appropriate, issued to each participant. Additionally, participants in extramurally sponsored activities involving inherent or potential hazards and/or dangers, should also complete the following:
A. Consent, Waiver, Release and Indemnity Agreement and Medical Consent Form – Adult; and/or
B. Consent, Waiver, Release and Indemnity Agreement and Medical Consent Form – Minor.
Oftentimes projects make arrangements for the chartering of transportation, such as buses, on behalf of project volunteers and/or other participants. If non-RCUH or non-UH employees are participating in the charter, it is recommended that the above-referenced agreement and form be obtained from these individuals.

II. Responsibilities
A. Principal Investigator
Ensure that participants are informed of the risks and rules of conduct, and that the Assumption of Risk, Release, and Indemnification Agreement and Medical Consent Form are completed and obtained from participants, if warranted by the nature of the sponsored activity.
