The W-2 forms for the 2009 tax year have been mailed out to RCUH and Maui High Performance Computing Center employees today.

If you do not receive your W-2 form by Saturday, January 30, 2010, please check with your local post office first to see that it is not being held up there.  If you have checked with your local post office and still have not received your W2, please contact [email protected].

Lost or Replacement Cost:  Please be advised that there is a $10.00 administrative fee, per copy, to reissue a W-2 form for any reason, including non-delivery of the initial W-2 form.
