Email or mailed submittals will be accepted (email is strongly preferred), but regardless of the submittal method, it is the Offeror’s responsibility to ensure confirmation of proposal receipt prior to the Closing Date for Receipt of Proposals.  Proposals received after the time and date fixed for submittal will not be considered.  Please refer to the Administrative Overview section in this RFP for other related key dates.

2010 RCUH STATEWIDE UNITED WAY CAMPAIGN

Welcome to this year’s RCUH United Way Campaign!  During these tough economic times we humbly ask for your continued support.  As one of the largest non-profit organization in Hawaii, the United Way supports a network of member agencies to meet a broad range of health and human service needs.

The RCUH Statewide United Way campaign allows our employees to donate to the various island wide United Way programs (Aloha United Way (Oahu), Hawaii Island United Way, Maui United Way, and Kauai United Way). 

Thanks to RCUH’s contributions, last year over $9,500.00 was donated to the various islands United Way program.  This year our goal is to raise $10,500.00 Statewide.

HOW TO CONTRIBUTE:  To contribute to the various Statewide United Way programs, complete the 2010 RCUH Employee Pledge Form and return the original form to RCUH Human Resources at 2530 Dole Street, Sakamaki Hall D-100, Honolulu, HI 96822.  Deadline:  Friday, October 1, 2010

Contribute via Payroll Deduction:  RCUH employees have the option to contribute to the various Statewide United Way Campaigns via payroll deduction. Your payroll deduction will be collectively sent to the United Way Campaign specified on your pledge form.  Payroll deduction period is from December 16, 2010 – December 15, 2011.

 

Optional Donor Choice Forms are available for you to designate a particular agency for your contributions to go to.  See the individual island campaign websites for more information & to obtain the Donor Choice Form.

      I.        FOR PROJECT SUPERVISORS & ADMINISTRATIVE STAFF:  We will be holding a one (1) hour Benefits Training Session for RCUH project supervisors and administrative staff on Kauai.

DEADLINE – SEPTEMBER 3, 2010 – STUDENT DEPENDENT HEALTH RENEWAL

The RCUH requires proof of status for any dependent between the ages of 19-24 each Spring and Fall semester after the dependent reaches the age of 19.  Certification may include a copy of the dependent’s college registration slip or class schedule. The registration slip/Class schedule must clearly indicate the student’s name, name of the accredited college/university, specific semester, and full-time status (defined as taking 12 or more credit hours per semester).  The student certification must be received by RCUH HR by THURSDAY, September 3, 2010.  Certification may be faxed to (808) 956-5022 (Please attention to Kristen Stevens for prompt review and to ensure that coverage is not cancelled).  Failure to submit the certification will result in IMMEDIATE CANCELLATION of student coverage (NO Exceptions), effective October 1, 2010, with the option of COBRA continuation effective October 1, 2010.  If you have any questions or concerns, please contact Kristen Stevens at [email protected].

DEADLINE – SEPTEMBER 30, 2010 – FLEXIBLE SPENDING PLAN REIMBURSEMENTS
The plan year July 1, 2009-June 30, 2010 for Flexible Spending Plans ended on June 30, 2010. You have 43 days left to submit reimbursement forms for services incurred during the plan year JULY 1, 2009 – JUNE 30, 2010 (Deadline: September 30, 2010). We encourage everyone to submit reimbursement forms and receipts now in the event that additional information is required. NO exceptions will be made if Benefit Services of Hawaii, Inc. (BSHI) does not receive ALL required documents by September 30, 2010.  Reimbursement forms and receipts should be mailed directly to BSHI (address is listed at the top of the reimbursement form). PRETAX TRANSPORTATION

Changes, cancellations and new enrollments (outside of initial hire) may be made with the submittal of a Pretax Transportation Benefits Change / Cancellation Form or RCUH Pretax Transportation Benefits Program Individual Enrollment Form to the RCUH Human Resources Department.  The enrollment / change forms must be submitted thirty (30) days PRIOR to the requested effective date of enrollment/change/cancellation.  For example, an employee who wants to make a plan change effective October 1, 2010 must submit his/her change form by September 2, 2010.  Forms may be downloaded from the 3.530 Flexible Spending Plan Policy – Pretax Transportation Addendum.  If you have any questions or concerns, please contact Kristen Stevens at [email protected].

Financial System Down Thursday, August 19, 2010, 5:00 pm through Monday, August 23, 2010, 8:00 am

The RCUH financial system will be disabled for system maintenance from 5:00 pm on Thursday, August 19, through 8:00 am on Monday, August 23, 2010.  Accordingly, Purchasing, Payments, UH-FMIS Fast-Track, and Accounting/Reporting applications will not be available during this time.  The Human Resources Portal will be available.
