POLICY UPDATE: RCUH 3.640 Sick Leave Policy

 

Effective May 1, 2012, the following additions will be made to the RCUH 3.640 Sick Leave Policy.

 

1.    Medical/Dental Appointments: With their Principal Investigator’s approval, employees taking leave to attend their own medical appointments, such as doctor’s visits, dentist’s visits, and medical testing may charge their leave as Sick Leave (previously required to charge appointments to Vacation or Leave Without Pay).   Doctor’s notes substantiating the medical appointments will be required.  Reference 4)c) of the “Details of Policy” section.

 

2.    Illness/Injury During Another Authorized Leave Period:  Employees who are already on another authorized leave status (e.g., vacation) will be allowed to charge their leave (or a portion of their leave) as Sick Leave if the employee has a serious illness/injury requiring hospitalization (doctor’s/hospital certification confirming hospitalization will be required).  Reference 10)d) of the “Details of Policy” section.

RCUH Benefits Open Enrollment Period 2012-2013

The Annual RCUH Benefits Open Enrollment Period (BOEP) is here.  BOEP Forms/Documents were sent out to all RCUH benefits-eligible employees yesterday, April 23, 2012, via email.  If you have not received your packet, please call Kelsey Milliken at (808) 956-7106 or email her at [email protected] to request for a duplicate packet.  Please review ALL of the materials provided.

Your Open Enrollment packet included the following: 

  • Informational Sheet (Document #1) Provides instructions to help you through the Open Enrollment Period. Includes important changes that will affect your benefit elections for the upcoming plan year (e.g., Flexible Spending Plan maximums, temporary enhancements to the dental plan etc.). 
  • Personalized Benefits Statement (Document #2) Lists employee’s current benefits as of April 23, 2012.
  • Fillable Enrollment/Change Form  (Document #3)
  • Medical/Dental Plan Summaries for Kaiser, HMSA & HDS plans (Documents #4-6) Highlights benefits/services covered/coverage levels and new rates as of July 1, 2012.
  • Flexible Spending Plan Highlights (Document #7) Highlights Flexible Spending Plan benefits and changes for the plan year, July 1, 2012 – June 30, 2013. 

During this Open Enrollment period, you can:

  • Enroll into a health insurance plan for employees who previously waived coverage (submit Document 3);
  • Add eligible dependents not already on your health insurance plan (submit Document 3);
  • Cancel/Change your health (medical & dental) insurance plan (submit Document 3);
  • Enter into an annual *Flexible Spending Plan for the Plan Year 7/1/2012-6/30/2013 *Even though you are currently enrolled, you must submit Document 3 if you want to participate in this benefit during the next plan year 7/1/2012-6/30/2013;
  • Enroll into, cancel or change Supplemental/Voluntary Insurance Plans offered through RCUH (submit Document 3). 

**REMINDER – You do NOT need to submit Document 3 if you are not newly enrolling into or making any changes to your medical/dental/voluntary insurance plans, and if you are not enrolling into a flexible spending plan for the upcoming plan year (7/1/2012-6/30/2013). 

 

 IMPORTANT BENEFIT ANNOUNCEMENTS   

 

  • NEW Maximum Annual Election Amount for Medical Expense Reimbursement Flexible Spending Accounts

In the upcoming plan year, July 1, 2012 – June 30, 2013, employees will be able to deduct a maximum of $2,500 per plan year for the Flexible Spending-Medical Expense Reimbursement Account (previously $2,400 for less than 4 years of service and $4,500 for 4 or more years of service).  This modification to policy is due to the enactment of the Patient Protection and Affordable Care Act (PPACA), which amended Section 125 of the Internal Revenue Code limiting the maximum Flexible Spending-Medical Expense Reimbursement Plan annual election to $2,500 per year.  See Document #7 for more information on the Flexible Spending Plan benefit.

 

  • Temporary Dental Plan Benefit Enhancements

We are pleased to announce that for the upcoming plan year, July 1, 2012 – June 30, 2013 only, rates will remain the same (no increase).  Additionally, RCUH will be temporarily increasing the benefit coverage levels for both Bitewing X-rays and Cleanings from 90% to 100%. Continuation of these temporary benefit enhancements into future plan years will be determined on a year-to-year basis.

 

  • RCUH Benefits Open Enrollment Meetings

We will be having three (3) Open Enrollment benefits meetings this year. The meetings will be held on Oahu (University of Hawaii at Manoa Campus) and broadcasted out via Hawaii Interactive Television System (HITS) to various sites on the outer islands. Representatives from our health/flexible spending vendors will be available to answer your questions, so we encourage all employees to take the time to learn your benefits options to ensure you are electing the best plan(s) to meet your needs.

FLEXIBLE SPENDING PLAN – Plan Year July 1, 2011-June 30, 2012 Reimbursements

 

2 MONTHS LEFT…to utilize your current Flexible Spending Plan Accounts (Medical Expense Reimbursement & Dependent Care Expense Accounts).  The end of the current Flexible Spending Plan Year is quickly approaching (June 30, 2012).  Services must be incurred during the plan year ending June 30, 2012.   Unused contributions will be forfeited at the end of the plan year (NO EXCEPTIONS). 

 

 

 

You may contact National Benefit Services (NBS) at 1(800) 274-0503 to obtain an updated status of your account.  Claims for reimbursements can be faxed (800/478-1528), mailed (National Benefit Services, LLC P.O. Box 6980, West Jordan, UT 84084), emailed ([email protected]) or uploaded via the Participant Website at https://www.nationalbenefitservices.com/.  

 

 

 

We urge all employees to submit their reimbursement forms to NBS in a timely manner.  Although services MUST be incurred by June 30, 2012, you have 90 days after this date to submit your reimbursement forms with ALL supporting documentation (ex. receipts, contracts) needed to reimburse you for Flexible Spending monies.  DO NOT WAIT until the end of the 90-day grace period to submit your reimbursement forms!!! STUDENT EMPLOYMENT

 

All RCUH Student Assistant employees are responsible for submitting a Student Verification each Semester in order to qualify for continued student employment.  For all University of Hawaii students (eg: UH Manoa, UH Hilo, Honolulu Community College, Maui College, etc.), verification is not required, as RCUH has the ability to verify student status´ electronically through a UH database.  However, verification is still needed for UH Outreach College and non-UH Schools (i.e. High School, Chaminade, HPU, etc).

 

 

 

The following are acceptable forms of student verification:

 

a) Student registration or receipt

 

b) Letter of student registration from the registrar or counselor

 

c) Validate student I.D. card

 

d) Any other form of verification deemed acceptable by the Director of Human Resources

 

 

 

Continuation of student employment:

 

Project/Student must submit Student Verification for the Fall 2012 semester by Tuesday, September 4, 2012.  Failure to submit student verification by the indicated deadline will result in termination of student employment.

 

 

 

Employment during summer break: 

 

Student Assistants who are enrolled for the Fall 2012 and are NOT taking any classes during the summer time, are eligible to work up to 40 hours per week.

 

 

 

Termination of student employment:

 

Student Assistants who no longer hold a student status must be terminated from RCUH due to their ineligibility (i.e. student graduation).  Principal Investigators must initiate a termination action via the HRAMP system by May 9, 2012.

SUMMER HIRING

 

Summer is the busiest season for the RCUH Human Resources Department.  In addition to being our peak hiring season, it also coincides with our Benefit Open Enrollment, Fiscal Year End, Annual Performance Evaluations and Pay Increases.

In anticipation of the peak hiring season, we ask all Principal Investigators and Project Administrators to review and adhere to the deadlines below to ensure that all hires are reviewed and approved without delay.  We appreciate your cooperation and assistance.

Selections for Regular Hires: minimum of 10 business days before target date of hire.

Non-Recruited Hires (Temporary, Specialized Service, Student Assistants, and Intermittent hire): minimum 7 business days before target date of hire.

Group Hires (6 or more employees): minimum of 15 business days before target date of hire.

Missing documents pertaining to the selection and/or incomplete new hire documents may cause the hire date to be delayed.  A formal offer of employment is made by the Principal Investigator only AFTER receiving approval to hire from the RCUH Human Resources Department.  Hire dates will be confirmed by our office via email.  All new hires & rehires are NOT approved to start working until their hire date has been confirmed by the RCUH Human Resources Office.

LIGHT DUTY ACCOMMODATIONS

 

If your employee requests for light duty accommodations, you, as the employee’s supervisor MUST coordinate the review with the RCUH Human Resources Department. A Job Physical Analysis (JPA) form must be completed and reviewed BEFORE the employee is allowed to return to work.  This is to ensure that the employee’s physician is fully aware of the employee’s essential job duties and functions before making a determination on whether the employee is able or unable to perform these duties/functions safely.  Additionally, as supervisor, you will need to review these detailed restrictions to determine if work accommodations (if any) can be made.

 

 

 

Supervisors are NOT authorized to allow an employee to return to work on light duty status without first receiving approval from the RCUH Human Resources Department. If you have any questions, please contact Jill Niitani at (808) 956-8376.

“LIGHT DUTY” REQUESTS

As a reminder, all requests for light duty accommodations MUST be coordinated with your supervisor and the RCUH Human Resources (HR) Department. A Job Physical Analysis (JPA) form must be completed and reviewed BEFORE you will be allowed to return to work.  This is to ensure that your physician is fully aware of your essential job duties and functions before making a determination on whether you are able or unable to perform these duties/functions safely. 

 

Employees are NOT authorized to return to work on light duty status without first receiving approval from the RCUH HR Department. If you have any questions, please contact Jill Niitani at (808) 956-8376.

 

3 MONTHS LEFT…

…to utilize your current Flexible Spending Plan Accounts (Medical Expense Reimbursement & Dependent Care Expense Accounts).  The end of the current Flexible Spending Plan Year is quickly approaching (June 30, 2012).  Services must be incurred during the plan year ending June 30, 2012.   Unused contributions will be forfeited at the end of the plan year (NO EXCEPTIONS).

 

We urge all employees to submit their reimbursement forms to National Benefit Services (NBS) in a timely manner.  Although services MUST be incurred by June 30, 2012, you do have 90 days after this date to submit your reimbursement forms with ALL supporting documentation (ex. receipts, contracts) needed to reimburse you for Flexible Spending monies.  DO NOT WAIT until the end of the 90-day grace period to submit your reimbursement forms!!!

 

In order to claim/submit for reimbursements, you need to complete the NBS reimbursement form and submit it along with a copy of your receipt(s) to “National Benefits Services” via fax, mail, or email:

The Annual Benefits Open Enrollment Period is just around the corner.  Notices/Forms will be sent out to all eligible employees in April, as well as posted on our website (available for download).  Please look out for special announcements regarding Open Enrollment next month.  As a reminder, benefit changes (e.g., enrollments, additions, etc.) made during the Open Enrollment period will be effective July 1, 2012.

If your employee requests for light duty accommodations, you, as the employee’s supervisor MUST coordinate the review with the RCUH Human Resources Department.
